Der Coenzym Q10-Mangel vom Typ 4 ist eine autosomal rezessive Erkrankung des zentralen Nervensystems und der Muskeln mit gelegentlicher Beteiligung von Herz und Nieren, die durch Mutationen im CABC1-Gen hervorgerufen wird.
Fünf verschiedene Phänotypen können unterschieden werden: (1) eine rein enzephalopathische Form mit Ataxie und Krämpfen, (2) eine multisystemische Form, wo neben der Enzephalopathie auch eine Kardiomyopathie und eine Nephropathie besteht, (3) eine zerebelläre Form mit Kleinhirnatrophie und Ataxie, (4) das Leigh-Syndrom mit Wachstumsretardierung und (5) eine isolierte Myopathie.
Bei manchen Patienten kann die Erkrankung erfolgreich mit Coenzym Q10 behandelt werden.
