Die Erkrankung wird durch Mutationen oder Imprintigfehler im GNAS1-Gen hervorgerufen. Das Syndrom besteht in Pseudohypoparathyreoididmus und verschiedenen Skelettveränderungen.
Die klinischen Symptome der Albright-Osteodystrophie (AHO) sind der Pseudohypoparathyroidismus (PHP) (periphere PTH-Resistenz) und Störungen der Skelettentwicklung (Kleinwuchs, Adipositas, Facies lunata, membranöse Ossifikationen, Brachydaktylie und andere). Die periphere PTH-Resistenz äußert sich vor allem in einem niedrigen Calcium- und erhöhten Phosphat-Spiegel, die sich durch Application von Parathormon nicht beeinflussen lassen, denn weder steigt nach PTH-Gabe die Calciumfreisetzung aus den Knochen, noch wird die renale Phosphatexkretion beeinflusst.
Das Imprinting dieses Gens ist dafür verantwortlich, dass die Erkrankung nur dann auftritt, wenn sie von einer gering betroffenen Mutter vererbt wird oder wenn eine Neumutation das mütterliche Allel betraf. Wird das erkrankte Allel vom Vater ererbt, so können sich auch einige der für AHO typischen Skelettveränderungen entwickeln. Aber die Laborwerte (Calcium und Phosphat) bleiben dabei normal. Deshalb wird dieser zustand Pseudopseudohypoparathyroidismus (PPHP) bezeichnet.
